Why are my fish dying when water parameters are OK?

Ammonia and nitrite at zero usually means the cycle is fine — but fish can still die from disease, aggression, chlorine, low oxygen, or temperature shock. High nitrate, forgotten dechlorinator, or a sudden temp change during a water change can all cause problems.
